Know what deserves attention before survey day.

The public trail
Your next survey isnot starting from zero.
CMS already holds signals about identity, deficiencies, penalties, staffing, ownership, and prior survey cycles. Survey Command coordinates them around one facility without pretending the public record is the whole truth.

Evidence before commitment
Inspect the brief. Then inspect the claim.
The governed Merry Wood Lodge demonstration artifact is available before commitment. Move through all three pages to inspect the executive read, ordered review, evidence register, and explicit claim boundary.

From evidence to leadership movement
A readiness brief should createa decision, not another report.
Survey Command carries the evidence into one executive working session. The team leaves with the same Current Reality, the same boundary, and one issue an accountable owner can investigate next.
What is the strongest supported reading of the published CMS record?
What leadership receives
One facility.
One consequential question.One executive record.
The engagement is deliberately narrow. It establishes a public starting reality leadership can inspect and use, without forcing an integration project before value is visible.
- 01Predict the next survey
- 02Replace clinical or legal judgment
- 03Claim current conditions from public data
- 04Require PHI or EHR access to begin

Direct answers
Beforeyou begin
01What is Survey Command?
A one-facility nursing home survey readiness assessment that coordinates the public CMS record into one source-bound Current Reality.
02Why is this not a mock survey or dashboard?
A mock survey simulates an inspection and a dashboard displays changing data. Survey Command establishes a dated operator read, preserves the evidence and unknowns behind it, and uses an executive session to decide what leadership should investigate next.
03Does Survey Command predict the next survey?
No. It identifies evidence-backed posture and investigation priorities. It does not predict inspection activity or guarantee an outcome.
04What does Credalyst need to begin?
The facility CCN, one operating question, and an executive owner. The first read requires no PHI or EHR integration.
05What does leadership receive?
An Executive Survey Brief, a leadership alignment session, portal access, and one evidence-justified first movement.
06How does it begin, and what happens afterward?
After scope and a signed agreement, Credalyst confirms the CCN and operating question and begins the evidence review. After the session, the issued brief and decision context remain in the Portal; Continuous Monitoring can keep the record current under a separate agreement.
